<p align="justify"> 8、哪些疾病可以办理特殊病种门诊治疗申请？如何办理？</p>
<p align="justify"> 答：以下十个病种列入特殊病种管理范围：1、恶性肿瘤的治疗；2、慢性肾功能衰竭的腹膜透析、血液透析；3、器官移植的抗排异治疗；4、心脏手术后抗凝治疗；5、系统性红斑狼疮（有心、肺、肾、肝及神经系统并发症之一者）；6、再生障碍性贫血；7、重性精神疾病；8、慢性乙型肝炎（活动性乙型肝炎）、乙型肝炎肝硬化、慢性丙型肝炎；9、糖尿病；10、血友病。</p>
<p align="justify"> 参保人员患有特殊病种确需进行门诊治疗的，原则上自行选择一到两家定点医疗机构作为特殊病种门诊定点就医医疗机构。参保人员（或委托代理人）到定点医疗机构医保办或柯城区社保局领取并填写 《衢州市柯城区基本医疗保险特殊门诊治疗申请表》，经医疗机构出具证明后，持《申请表》和就诊资料（住院或门诊病历复印件，支持特殊病种诊断的相关检查或治疗报告等）到区社保局办理申报核准手续。特殊病种门诊定点就医经核准后生效，原则上一年内不办理变更。</p>
